Help Shape Our Community’s Response to Substance Use: Please Take Our Survey

We want to hear from you!  We are conducting a short, confidential survey to better understand substance use and its impacts on Lake County residents.  Lake County is receiving opioid settlement dollars over the next 18 years through the National Opioid Settlement, and we value your input in helping us determine how the county should invest this funding in our community.  Your input will help guide future programs and resources to address our community's greatest needs to support individuals and families impacted by substance use.

🕒 It only takes a few minutes!
