Parents or guardians of students enrolled in the School City of Hobart schools must download and complete the necessary insurance consent forms for their children to receive required school vaccinations at the Hobart High School Vaccination Clinic in Hobart.

There are two forms available: one for students covered by private insurance and another for those with any other type of insurance.

To ensure their child’s participation, parents must fill out the appropriate form and submit it via email no later than May 29, 2025.
